Title: Case Manager
Reports To: Psychosocial Services Coordinator or/ Program Director / Regional Director of Psychosocial Services
Location: 16 East 16th Street, New York, NY 10003
Summary: Works as part of interdisciplinary team to help patients and families with personal and
environmental difficulties, which predispose illness, of interfere with obtaining maximum
benefits from medical care.
A. Clinical/Technical/Service N/A
? Demonstrates the ability to perform clinical service and administrative tasks.
? Works in close collaboration with physicians and other health care personnel in patient evaluations and treatment to further their understanding of significant social and environmental factors underlying patient?s health problems.
? Prepares case management assessments, patient histories, service plan and reports.
? Develops linkages with community agencies as needed for funding sources, enhancing patient care or promoting the development of social services and the Institute as a whole.
? Provide case management services to assist patients in receiving necessary resources and services. Medicaid, Child Health Plus, Welfare, DAS, Social Security, Child Care, Home Care, Housing/Shelter/Safe Homes, Substance Abuse Treatment, Mental Health Treatment, HIV Services, Legal, etc.
? Work with providers to assess child abuse and neglect and follow guidelines for reporting when necessary; provide follow-up support services to families to maximize family functioning.
? Document all patient services in patient?s electronic medical record, closes encounters within 48 hours.
? Follow all policies and procedures of Department of Psychosocial Services..
? Provides monthly statistical reports to administration.
? Participates in all required staff meetings.
? Demonstrates the knowledge and skills necessary to provide care, based on physical, psychosocial, educational, safety, and related criteria, appropriate to the age of the patients served in assigned area.

E. Documentation Section
? Enter all encounters into electronic medical record system
? Ensures all patients are ?arrived? in electronic medical record system
? Completes psychosocial assessment on all new patients on first visit
? Accurately records level of service in electronic medical record system
? Documents all referral services
? Obtains patient consents to coordinate with all referrals and services
? Accurately completes patient logs
? Completes statistical data in a comprehensive and timely manner
? Follows agency and funder guidelines for outreach
? Documents all outreach attempts to patients and collaboration with health care team
? Provides feedback to referral sources
? Closes all patient care encounters within 48 hours
? Responds to all referral requests within 48 hours
? Provide health education information and resources
F. Practice Section
? Appropriately refers patients for mental health services
? Appropriately advocates for patients with outside systems
? Identifies and reports suspected child abuse, neglect and maltreatment
? Follows agency policy surrounding the identification and reporting of child abuse
? Collaborates with agency staff
? Identifies service gaps and seeks solutions
? Creates outside linkages
? Identifies and utilizes community resources
? Meets agency and funder visit targets
? Accurately conducts patient resource assessments
? Notifies supervisor of all home and off site patient visits
? Does not conduct home or off site patient visits alone
? Treats patients with respect
? Is empathic
? Engages patients into care
? Appropriate refers patients to pharmacy programs.
? Accurately reviews patients income and adjusts patient type
EDUCATION REQUIRED
? Bachelor?s degree in Social Work, Human Services or related field.
EXPERIENCES AND/OR SKILLS REQUIRED
? Basic knowledge of computers, data entry experience a plus.
? Ability to write reports and correspondence.
? Knowledge of Social Services agencies and resources in the NYC area
? Healthcare experience preferred.
? Ability to work independently and as part of a team
? Bi-lingual, Spanish preferred.
